Students in 6-12 that are participating in sports or NSAA activities need a physical and athletic trainer waiver. Students in 9-12 need to complete a NSAA form also. Parents/Guardians make sure you sign the forms as well. Please turn in before 8-11-25.

Concussion Baseline Testing: August 11,2025 @ 1:30 pm at Stapleton School. Please bring cell phone. All students in grades 6-12 planning on doing any sports need to complete a baseline test.
Junior high, if you are unable to attend, an alternative date will be provided.
